New clients scheduling Reiki, Raindrop Therapy, Crystal Healing Bed or any other Sessions

are required to complete the Client Intake and Liability Waiver forms.  

Please take a few minutes to complete these forms prior to your initial appointment.

Client Intake Form: 

Pull up the Client Intake form which is an online form. 

Complete each section, select the submit button, and your information will be submitted online.

Liability Waiver Form: 

Pull up the Liability Waiver form, read thoroughly, complete the Client Authorization & Signature section,

select the submit button, and your information will be submitted online. 

For clients who prefer retaining a hard copy, simply copy the document from the same page and print.
